For additional information, contact Member Services at 1-866-431-1687.  To locate a provider, visit www.thcplus.com, click on the “Find a Provider” link and you will be directed to Chiropractic Lifecare of America.  When making an appointment, identify yourself as a member of the Chiropractic Lifecare of America network and have your ID card with you at the appointment.
