Emergency Travel
Overview
Emergency Travel will reimburse you for unexpected expenses if your vehicle is disabled when you are more than fifty (50) miles from home. If you are more than fifty (50) miles from your primary residence and your vehicle is required to be in a repair facility for more than twenty-four (24) hours due to a collision or mechanical failure that prevents your vehicle from being driven, we will reimburse you for the actual charge for your hotel stay and meal costs you incur while your vehicle is in the repair facility. If your vehicle is disabled due to a collision, you must report the collision to the police and obtain a police report within 72 hours of the collision.

This summary is a brief overview of the Emergency Travel benefit. The Emergency Travel coverage benefit is underwritten by Virginia Surety Company, Inc. under group insurance policy form number GEN-GP(8.07). Exclusions and limitations apply. An Evidence of Coverage will be sent to you when you enroll in the membership plan. Please refer to the Evidence of Coverage for complete details of the terms, conditions and exclusions. The Emergency Travel coverage benefit is not available to residents of the state of Texas.

DEFINITIONS:
Throughout this document, “You” and “Your” refers to the person who is a member in good standing in 4U Benefits Membership Program as defined by the terms and conditions for 4U Benefits Membership Program must not have expired or been canceled by the You or 4U Benefits Membership Program. “We”, “Us”, and “VSC” refer to Virginia Surety Company, Inc. In addition, when in bold certain words and phrases are defined as follows:
Administrator means cynoSure Financial, Inc. You may contact the Administrator if you have questions regarding this coverage or would like to make a claim. The Administrator can be reached by phone at 800-711-4280.
Common Carrier means any regularly scheduled air or land motorized transportation carrier operating under a regularly published schedule and current license as required by law for the conveyance of passengers when the appropriate fare or charge is paid. Common Carrier does not include helicopters,taxis, rental cars,hired cars,and private and contract carriers.
Coverage Period means the period starting thirty (30) days from on the Membership Effective Date. Coverage will continue for Emergency Travel. Coverage will continue as determined by the payment schedule/plan for as long as You are a member in good standing. (This standing is defined by the terms and conditions for 4U Benefits Membership Program. ) Coverage will stop if You or 4U Benefits Membership Program cancel Your membership in 4U Benefits Membership Program.
Domestic Partner means an unmarried person in an intimate, committed relationship of mutual caring. They must share responsibility for basic living expenses with You. They must be at least eighteen (18) years old and not currently married and/or committed to another person.
Evidence of Coverage (EOC) means this document. It describes the terms, conditions, and exclusions. The EOC is the entire agreement between You and Us. Representations or promises made by anyone that are not contained in this EOC are not a part of Your coverage.
Family Member means Your spouse or Domestic Partner. Family Member includes unmarried children under nineteen (19) years of age. Family Member also includes unmarried children under [twenty-three (23) years of age if a full-time student at an accredited college or university. Any Family Member who does not reside at Your home is not eligible for coverage. The Family Membership option in 4U Benefits Membership Program must be purchased for coverage to apply to Family Members.
Membership Effective Date means the date You enroll as a member in 4U Benefits Membership Program or upon receipt of payment of Your initial membership dues as per the membership terms and conditions for 4U Benefits Membership Program, whichever occurs last.
Primary Residence means the home that You live in at least nine (9) months out of the year.
Travel Expenses means the following expenses incurred while Your vehicle is being repair as a result of a Vehicle Disablement:
1) Food and lodging expense: The actual charge for overnight hotel stay and meal costs incurred by You.
2) Rental vehicle expense: The actual cost charged by the rental agency for the rental of a private-passenger vehicle.
3) Transport of vehicle expense: The actual cost charged by a car shipping service to transport Your vehicle back to Your Primary Residence. 4) Common carrier ticket expense: The actual cost for a Common Carrier ticket to return to Your Primary Residence.
Vehicle Disablement means a collision or mechanical failure that prevents Your vehicle from being driven.
B. COVERAGE
Emergency Travel
If during the Coverage Period You are more than fifty (50) miles from Your Primary Residence and Your vehicle is required due to a Vehicle Disablement to be in a repair facility overnight for more then twenty-four (24) hours, we will reimburse You for Travel Expenses that are incurred as a direct result of Your vehicles Vehicle Disablement.
You must report and obtain a to the police report within seventy-two (72) hours of the collision, resulting in Your vehicles Vehicle Disablement.
Limitations:
• Coverage is limited to $500 per Vehicle Disablement.
• Coverage is further limited to:
o Food and lodging expense benefit up to $100 per day, payable for up to 5 days.
o Rental vehicle expense benefit up to $100, payable for up to 5 day limit.
o Transport of vehicle expense benefit up to $100.
o Common Carrier ticket expense benefit up to $100.
• Coverage is limited to 2 claim(s) per membership period per twelve (12) month period.
C. EXCLUSIONS
Coverage does not apply to:
• Vehicle Disablement that occurred prior to the start of the Coverage Period.
• Any act of theft, deceit, collusion, dishonesty or criminal act by You or any person acting in concert with You, or by any authorized representative of You, whether acting alone or in collusion with You or others.
• Acts of God.
• Theft or vandalism of Your or Your Family Members vehicle.
• Commercial or non private-passenger vehicles.
• Business or commercial travel.
• Mechanical failure of the vehicle.

How To File a Claim
How to file a claim:
To file a claim, You must contact the Administrator by phone within fifteen (15) days of the date of the Vehicle Disablement. Otherwise, the claim may be denied.
A claim form will be sent to You. The fully completed and signed claim form must be returned to the Administrator with:
1.Completed and signed claim form.
2.Proof of the Vehicle Disablement.
3.A police report, if the Vehicle Disablement was due to a collision.
4.For Emergency Travel – Repair statement indicating that Your vehicle was kept in the repair facility overnight for more than twenty-four (24)] hours.
5.For Destination Assistance – a copy of the Common Carrier receipt.
6.Any other documentation that may be reasonably requested to validate a claim.
All these required items, including the claim form, must be postmarked within ninety (90) days of the date of the Vehicle Disablement. Otherwise, the claim may be denied.
